Online Housing Application

The Disability Programs and Resource Center (DPRC) works in collaboration with the SF State Student Housing Program to provide reasonable accommodations within our residential facilities for students with disabilities. Note that you must apply for Student Housing through the SF State housing portal and also request a housing accommodation through DPRC. Therefore, to initiate the housing accommodation process, you need to both apply with housing and complete the form below. We also recommend you review the DPRC documentation guidelines.

By completing this form, I authorize the DPRC to interact on my behalf, and to exchange information as necessary with the University Housing Program, and if needed, my care provider, in order to document need for my disability-related housing accommodation request.
